Completing the Nomination Form : As a general rule, the nominator should limit documentation to no more than four, single-spaced typewritten pages (or equivalent, if handwritten). There are two critical considerations for the nomination materials: (1) Be responsive to the criterion for the award ("a record of outstanding achievements in a career and/or civic involvement"); and (2) Remember that selection committee members will make their determinations based on the responsiveness, clarity and completeness of the nomination materials. As a nominator, you have decided to offer this person for consideration; do make the very best "case" that you can.

Provide information on the following:
1. Summary Statement . In one page or less summarize the most important aspects of the nominee’s life and/or career that qualify him/her for this award. (You will have space for elaboration in numbers 2 and 3 below.)

2. The Record. Describe the nominee’s record of outstanding service in civic, philanthropic or volunteer activities and/or record of outstanding achievements in the nominee’s chosen career (significant career accomplishments including publications, research, patents, other creative work and/or involvement in professional societies).

3. Recognition . Identify and describe honors and awards that the nominee has received.
