Alumni Achievement Award Nomination Form 
The Award : Established in 1974 for alumni who have a record of outstanding achievements in a career and/or civic involvement. The alumni association presents the award to no more than six graduates at the All-University Celebration and Salute to Alumni Achievement the night before May graduation. (The date of the degree determines class membership. If the individual did not receive a degree, the nominee shall be considered a member of the class that graduated four years after first enrollment. If the nominee has received more than one NU degree, the individual shall select one degree date for award purposes.)

- Recipients MUST attend the awards ceremony to receive an award.
- The fact that an individual has previously received an alumni association award in another category does not preclude him/her from receiving this award.
- The awards committee will accept nominations from any alumnus, friend or alumni affiliate organization of the University of Nebraska-Lincoln.
- The awards committee retains and considers nominations for three years or until selected.
- The deadline for submitting nominations for the 2009 awards is Dec. 1, 2008.
